Welcome to UltraCare Transport. By accessing our website or using our non-emergency medical transportation (NEMT) services, you agree to be bound by these Terms of Service (“Terms”). Please read them carefully. If you do not agree to these Terms, do not use our services.

1. Services Provided

UltraCare Transport provides non-emergency medical transportation services in the Dallas-Fort Worth metropolitan area. Our services include ambulatory transportation, wheelchair-accessible transportation, and dialysis transportation, among others. We do not provide emergency medical services or ambulance transport.

2. Eligibility

Our services are available to individuals 18 years of age or older, or to authorized caregivers, guardians, or facility staff booking on behalf of a patient. By using our services, you represent that you have the authority to agree to these Terms on behalf of yourself or the individual you represent.

3. Booking and Scheduling

  • All trip requests must be submitted through our website, by phone, or through an authorized broker or facility.
  • Trip confirmation is not guaranteed until you receive an explicit confirmation from UltraCare Transport.
  • We reserve the right to decline or cancel a trip request for any reason, including capacity limitations, unsafe conditions, or inability to safely accommodate a passenger’s needs.
  • For recurring trips (e.g., dialysis), scheduling terms will be confirmed in writing at the start of service.

4. Cancellation and No-Show Policy

  • Cancellations: Please notify us at least 2 hours before your scheduled pickup time. Late cancellations may be subject to a cancellation fee.
  • No-Shows: If our driver arrives and cannot reach the passenger after reasonable attempts, the trip may be marked as a no-show and a fee may apply.
  • Repeat no-shows may result in suspension of service.

5. Payment Terms

  • Payment is due at the time of booking unless a facility billing arrangement has been established in writing.
  • We accept major credit and debit cards through our secure payment processor, Stripe.
  • All rates are subject to change. Trip pricing is confirmed at the time of booking.
  • Refunds will be issued at our discretion for trips cancelled in accordance with our cancellation policy.

6. Passenger Conduct

Passengers and their attendants are expected to treat our drivers and vehicles with respect. UltraCare Transport reserves the right to refuse service or terminate a trip for any passenger who:

  • Engages in threatening, abusive, or disruptive behavior
  • Causes damage to our vehicles
  • Violates any applicable law during transport

7. Medical Limitations

UltraCare Transport provides transportation only — we are not a medical provider. Our drivers are not licensed medical professionals and are not authorized to provide medical advice, administer medication, or make medical decisions on your behalf. If a passenger requires active medical monitoring during transport, they must be accompanied by a qualified medical professional.

8. Limitation of Liability

To the fullest extent permitted by law, UltraCare Transport shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from the use of our services, including but not limited to missed medical appointments, delays due to traffic or weather, or third-party actions. Our total liability to you for any claim arising from our services shall not exceed the amount you paid for the specific trip giving rise to the claim.

9. Indemnification

You agree to indemnify and hold harmless UltraCare Transport, its owners, employees, drivers, and subcontractors from any claims, damages, losses, or expenses (including reasonable attorney’s fees) arising from your use of our services, your breach of these Terms, or any misrepresentation you make in connection with booking our services.

10. Insurance

UltraCare Transport maintains appropriate commercial auto and general liability insurance. However, our insurance does not cover personal property of passengers, and we recommend passengers secure their own insurance for valuable items.

11. Intellectual Property

All content on ultracaretransport.com, including text, graphics, logos, and page layouts, is the property of UltraCare Transport and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, or create derivative works from our content without our express written consent.

12. Privacy

Your use of our services is also governed by our Privacy Policy, which is incorporated into these Terms by reference.

13. Modifications to Terms

We reserve the right to update these Terms at any time. Changes will be effective upon posting to our website. Your continued use of our services after changes are posted constitutes acceptance of the revised Terms.

14. Governing Law

These Terms are governed by the laws of the State of Texas, without regard to its conflict of law provisions. Any disputes arising from these Terms or our services shall be resolved in the courts of Dallas County, Texas.
